Mais de 700 mil pessoas morrem todos os anos devido à resistência antimicrobiana, causada pelo uso indevido ou prescrição de antibióticos, o que faz com que as bactérias se adaptem e resistam cada vez mais aos medicamentos.
Segundo a OMS, os antibióticos são medicamentos utilizados para prevenir e tratar infecções bacterianas e a resistência aos antibióticos ocorre quando as bactérias sofrem mutação em resposta ao uso destes medicamentos.
São as bactérias, e não os humanos ou os animais, que se tornam resistentes aos antibióticos. Estas bactérias resistentes aos medicamentos podem causar infecções em humanos e animais, e estas infecções são mais difíceis de tratar do que as não resistentes.
A resistência aos antibióticos causa aumento dos custos médicos, internações hospitalares prolongadas e aumento da mortalidade.
Escopo do problema
A resistência aos antibióticos está a aumentar em todo o mundo para níveis perigosos. Dia após dia, novos mecanismos de resistência surgem e espalham-se por todo o planeta, comprometendo a nossa capacidade de tratar doenças infecciosas comuns. Um número crescente de infecções, como a pneumonia, a tuberculose, a septicemia, a gonorreia e as doenças de origem alimentar, estão a tornar-se mais difíceis – e por vezes impossíveis – de tratar à medida que os antibióticos se tornam menos eficazes.
Onde os antibióticos podem ser adquiridos sem receita médica para uso humano ou veterinário, o surgimento e a propagação da resistência aos medicamentos pioram. Em países que não possuem orientações terapêuticas padronizadas, os profissionais de saúde e veterinários tendem a prescrevê-los – e a população em geral a consumi-los – em excesso.

O que podemos fazer?
Embora saibamos que a resistência bacteriana é um processo natural e não pode ser interrompido, podemos contê-la. Em Espanha temos o Plano Nacional contra a Resistência aos Antibióticos (PRAN), com duas estratégias: reduzir o consumo de antibióticos e reduzir a necessidade da sua utilização na medicina humana e veterinária. Como? Com essas seis diretrizes que contamos a seguir.
1. Não se automedique
Você nunca deve guardar sobras de antibióticos. Se um familiar que teve uma infecção na garganta recebeu um determinado medicamento que sobrou, você nunca deve guardá-lo e usá-lo para possíveis infecções que possam surgir no futuro. É quase certo que você pode confirmar que não será o caminho certo para aquela pessoa. Não devemos esquecer que dos quatro milhões de consultas anuais em Espanha por dor de garganta, 70% são devidas a causas virais, que geralmente se resolvem, na sua maioria, por si mesmas.
2. Prescreva o medicamento certo
Existem antibióticos de amplo espectro que certamente matam uma infecção de dor de garganta, mas pode não ser necessário tomá-los para matar a bactéria que causa a infecção. Usando um antibiótico de amplo espectro para matar a bactéria que causa a amigdalite, a única coisa que consegue é que essa bactéria se torne resistente.
3. Verifique se há infecção bacteriana
80% dos antibióticos que tomamos são prescritos na Atenção Básica e é o médico quem se encarrega de dar o diagnóstico correto e confirmar se a infecção é causada por vírus ou bactéria. Isto é essencial, uma vez que os antibióticos não são eficazes contra os vírus: não impedem o seu crescimento nem os matam.
Daí a importância de submeter os pacientes a diagnósticos microbiológicos. O ideal seria sempre fazer culturas (uma amostra do tecido infectado) e um antibiograma (exame laboratorial em que a bactéria retirada da amostra é exposta a diversos antibióticos para verificar se ela é sensível ou resistente a eles). Enquanto chega o resultado (não é imediato, demora alguns dias) costuma-se prescrever um antibiótico empírico, ou seja, um tratamento probabilístico que se inicia sem conhecer a bactéria causadora. Mudar para um de menor espectro, mas eficaz, é essencial para evitar resistências e, no caso de crianças, idosos ou imunossuprimidos, especialmente importante
4. Termine o ciclo ditado pelo protocolo
Muitas vezes alguém que sofre de angina toma o antibiótico por dois dias, mas quando se sente bem, para. Não deu tempo de matar as bactérias, então elas ficam ali, se multiplicando e gerando mais resistência. Tem-se falado muito sobre a duração ideal dos ciclos de tratamento com antibióticos, se devem ou não ser mais curtos - alguns estudos demonstraram que ciclos mais curtos podem ser tão eficazes como ciclos longos em algumas infecções - mas a última palavra sobre a duração do a escolha do antibiótico fica por conta do médico, que sempre possui dados clínicos atualizados para fornecer informações adequadas ao paciente.
5. Evite o uso em animais saudáveis
Os humanos não são os únicos que tomam antibióticos. A sociedade deve controlar e não consumir alimentos que utilizem antibióticos para promover o crescimento ou prevenir doenças em animais saudáveis. É por isso que a União Europeia proibiu o uso de antibióticos para promover o crescimento animal desde 2006. De acordo com a legislação europeia, apenas os veterinários podem prescrever os antibióticos a administrar aos animais e estabelecer a dosagem correta e a duração do tratamento.
6. Higiene e vacinação, medidas fundamentais
Lavar as mãos frequentemente com água e sabão e usar desinfetante para as mãos à base de álcool protege contra essas bactérias nocivas. Assim como hábitos saudáveis como seguir uma dieta adequada, manusear adequadamente os alimentos, desinfetar superfícies ou praticar atividade física também ajudam a reduzir o risco de desenvolver infecções. Por outro lado, embora saibamos que os antibióticos não tratam os vírus, é possível que uma infecção viral, como a gripe, acabe sendo complicada por infecções bacterianas secundárias para as quais são necessários antibióticos, como a pneumonia. A vacinação contra a gripe ou a varicela, que também é uma doença viral que pode causar infecções bacterianas da pele ou pneumonia, ajudará a reduzir o uso desses medicamentos.
