Más de 700.000 personas mueren cada año debido a la resistencia a los antimicrobianos, causada por un mal uso o prescripción de los antibióticos, que hace que las bacterias se adapten y resistan cada vez más a los fármacos.
Según informa la OMS, los antibióticos son medicamentos utilizados para prevenir y tratar las infecciones bacterianas y la resistencia a los antibióticos se produce cuando las bacterias mutan en respuesta al uso de estos fármacos.
Son las bacterias, y no los seres humanos ni los animales, las que se vuelven resistentes a los antibióticos. Estas bacterias farmacorresistentes pueden causar infecciones en el ser humano y en los animales y esas infecciones son más difíciles de tratar que las no resistentes.
La resistencia a los antibióticos hace que se incrementen los costos médicos, que se prolonguen las estancias hospitalarias y que aumente la mortalidad.
Alcance del problema
La resistencia a los antibióticos está aumentando en todo el mundo a niveles peligrosos. Día tras día están apareciendo y propagándose en todo el planeta nuevos mecanismos de resistencia que ponen en peligro nuestra capacidad para tratar las enfermedades infecciosas comunes. Un creciente número de infecciones, como la neumonía, la tuberculosis, la septicemia, la gonorrea o las enfermedades de transmisión alimentaria, son cada vez más difíciles —y a veces imposibles— de tratar, a medida que los antibióticos van perdiendo eficacia.
Allí donde los antibióticos se pueden adquirir sin receta médica para uso humano o veterinario, la aparición y propagación de la farmacorresistencia empeora. En los países que carecen de directrices terapéuticas normalizadas, el personal sanitario y veterinario tiene tendencia a prescribirlos —y la población general a consumirlos— en exceso.
Si no se toman medidas urgentes, el mundo está abocado a una era post-antibióticos en la que muchas infecciones comunes y lesiones menores volverán a ser potencialmente mortales.

¿Qué podemos hacer?
Aunque sabemos que la resistencia de las bacterias es un proceso natural y que no puede detenerse, sí podemos contenerlo. En España tenemos el Plan Nacional frente a la Resistencia a los Antibióticos (PRAN), con dos estrategias: reducir el consumo de antibióticos y disminuir la necesidad de usarlos en la medicina humana y en la veterinaria. ¿Cómo? Con estas seis pautas que te contamos a continuación.
1. No automedicarse
Nunca hay que guardar las sobras de los antibióticos. Si a algún miembro de la familia que tuvo infección de garganta se le recetó un determinado fármaco del que sobró bastante, nunca hay que guardarlo y aprovecharlo para posibles infecciones que se presenten en un futuro. Casi con total seguridad se puede confirmar que no será el adecuado para esa persona. No hay que olvidar que de los cuatro millones de consultas al año en España por dolor de garganta, el 70 % se deben a causas víricas, que se suelen curar, en su gran mayoría, solas.
2. Recetar el medicamento adecuado
Hay antibióticos de amplio espectro que con toda seguridad van a acabar con una infección de anginas, pero quizá no es necesario tomarlos para acabar con la bacteria que causa la infección. Utilizar un antibiótico de amplio espectro para acabar con la bacteria que provoca las anginas lo único que consigue es que esa bacteria se haga resistente
3. Comprobar si hay infección bacteriana
El 80 % de los antibióticos que tomamos se prescriben en Atención Primaria, y es el médico el encargado de dar el diagnóstico correcto y confirmar si la infección está causada por un virus o por una bacteria. Esto resulta fundamental, ya que los antibióticos no son eficaces contra los virus: ni frenan su crecimiento ni los matan.
De ahí la importancia de someter a los pacientes a diagnósticos microbiológicos. Lo idóneo sería hacer siempre cultivos (una muestra del tejido infectado) y un antibiograma (prueba en el laboratorio en la que se expone la bacteria que se ha tomado de la muestra a varios antibióticos para ver si resulta sensible o resistente a ellos). Mientras llega el resultado (no es inmediato, tarda unos días) se suele mandar un antibiótico empírico, es decir, un tratamiento de probabilidad que se inicia sin conocer la bacteria causante. Cambiar a uno con menos espectro, pero eficaz, es fundamental para evitar la resistencia y, en el caso de los niños, ancianos o inmunodeprimidos, especialmente importante
4. Acabar el ciclo que dicta el protocolo
Muchas veces alguien que padece anginas se toma dos días el antibiótico, pero al sentirse bien, lo deja. No ha dado tiempo a matar a las bacterias, por lo que se quedan ahí, multiplicándose y generando más resistencia. Se ha hablado mucho sobre la duración idónea de las tandas de tratamiento antibiótico, si deberían ser o no más cortas —algunos estudios han demostrado que ciclos más cortos pueden tener la misma eficacia que los largos en algunas infecciones—, pero la última palabra en cuanto a la duración del antibiótico la tiene el médico, que siempre cuenta con los datos clínicos actualizados para proporcionar al paciente la información adecuada.
5. Evitar su uso en animales sanos
Los humanos no somos los únicos que tomamos antibióticos. La sociedad debe controlar y no consumir alimentos que utilizan antibióticos para estimular el crecimiento o para prevenir enfermedades en animales sanos. Por eso la Unión Europea prohíbe desde 2006 el uso de antibióticos para favorecer el crecimiento de los animales. De acuerdo con la legislación europea, solo los veterinarios pueden prescribir los antibióticos que se administrarán a los animales y establecer la dosis correcta y la duración del tratamiento.
6. Higiene y vacunación, medidas clave
Lavarse las manos con agua y jabón de manera frecuente y usar desinfectante para las manos a base de alcohol protege de esas bacterias dañinas. Al igual que hábitos saludables como seguir una dieta adecuada, manipular alimentos como corresponde, desinfectar superficies o practicar actividad física también ayudan a reducir el riesgo de desarrollar infecciones. Por otro lado, aunque sabemos que los antibióticos no tratan los virus, es posible que una infección viral, como la gripe, termine complicándose con infecciones bacterianas secundarias para las que sí que se necesitan antibióticos, como una neumonía. Vacunar contra la gripe o la varicela, que también es una enfermedad vírica que puede desarrollar infecciones bacterianas en la piel o neumonía, servirá para reducir el uso de estos fármacos.

Respuesta de la OMS
La lucha contra la resistencia a los antibióticos reviste alta prioridad para la OMS. La Asamblea Mundial de la Salud aprobó en mayo de 2015 un plan de acción mundial sobre la resistencia a los antimicrobianos, incluida la resistencia a los antibióticos. Su finalidad es asegurar que se pueda seguir previniendo y tratando enfermedades infecciosas por medio de fármacos eficaces y seguros.
En la Asamblea General de las Naciones Unidas de septiembre de 2016, los Jefes de Estado se comprometieron a abordar de forma amplia y coordinada las causas profundas de la RAM en diferentes sectores, en particular los de la salud humana, la salud animal y la agricultura. La OMS está prestando apoyo a los Estados Miembros en la elaboración de planes de acción nacionales sobre la RAM basados en el plan de acción mundial.
